Nginx & php5-fpm - All pages 404 error, wordpress login loop, subscriber 500 server error

Hey all,

I'm running into a ton of issues nonstop for the last 7 hours and after playing with the settings and really trying to get this fixed, it just isn't happening. Am desperate as this is a live site with a decent amount of visitors.

Ubuntu 14.04 x64

  1. Only homepage & category pages are showing, rest of pages and posts are 404s.
  2. Wordpress login keeps looping (never allowing me into wp-admin)
  3. When someone subscribes to a newsletter, the post fails and hits 500 server error page

Here are two types of errors I'm getting:

I switched from apache to nginx a few days ago and for some odd reason things started breaking down over time.
2016/01/24 21:41:48 [warn] 12503#0: conflicting server name "" on, ignored

Additionally, it keeps trying to open old files that don't exist (plugins I deleted). That may be nginx caching? I cleared my cloudflare cache multiple times and I do have nginx caching set up. How would I get rid of cache?

I'll post whatever other files you need, just let me know.
