I’m working on a large app & website using PHP\Laravel, and I have the following servers:
However, I want to add a load balancer in front of the database to distribute the traffic.
My question is, will the managed database help with this? Or I should go with instances instead and configure the MySQL cluster manually?

You should be able to use both the suggested options - managed database, configuring MySQL cluster manually.
My opinion however would be to go about configuring it manually but that’s a personal reference not based on any facts, I just like doing it that way. Having said that, both options are plausible and achievable. It’s basically, up to you.
