Have you reviewed your log files? Any time you see an error returned by your web browser (as long is it hasn’t been reconfigured not to log) an entry will be created in your log file with more information. The default log file location for Apache is /var/log/apache2/error.log on Debian/Ubuntu and /var/log/httpd/error.log on CentOS/Fedora.
