I have a droplet running the automated version with Plesk 18.0 on Ubuntu 18.04 and already reached the limit of 3 domains.
Are there potential risk or problem if I manually setup a new website on the same droplet without using Plesk?

You will need to manually create the virtual host for the domain/site and edit the web server configuration file and this might get overwritten from Plesk when doing restarts or updating modules and plugins, which can give you a headache along the way. Rather than that it should be fine and not cause errors/issues to your other sites.
However keep in mind that this is not really recommended as issues might occur and your setup will become pretty much bespoke making it not that straight forward to maintain.
