You can configure an OpenVPN VPN pretty easily. There is a pretty good article about it from DigitalOcean:

The article itself covers most of the requirements you’ve given, those that are not covered in the article, you should be able to get from your Droplet as information from various services.
Having said that, can you let us know where exactly do you need to fill this up?
